Child and Family Services

Connect Youth and Family Services offers Child and Family Therapy Services to fit the needs of all types of patients and their families.  See each type of services below including: Therapeutic Day Treatment, Intensive In-Home Services, Home-Based Support, and Family Therapy Services.

*Approval of services are contingent upon prior authorization from Medicaid Managed Care Organizations and/or Comprehensive Services Act.

Therapeutic Day Treatment



Services are provided to school-aged children who have difficulty with emotional and behavioral stability in the school environment.

​Service Provides:​
​
  • A therapeutic environment in which an evidenced based curriculum is utilized 

  • Social-emotional learning, character building and psycho-educational activities through art, community outings, recreation and small groups 

  • Person-centered treatment and goal setting 

  • Individual and family counseling 

  • Service coordination with school and other providers 

  • Transportation, meals and a snack to all participants

​Criterion for Services:
​
  • Does your child have a history of behavioral challenges that have warranted  suspensions or multidisciplinary planning (child study, etc.) in the school setting? 

  • Does your child require individual and small group support to address their mental, emotional and behavioral issues that create a barrier in learning? 

  • Has your child ever had any prior history of mental health services? 

  • Does your child have difficulty getting along with others?

Services



Service provides children, adolescents and their families with short term,  goal directed-support within the home environment.

​Service Provides:
​
  • 24 hours crisis support and intervention to prevent hospitalization and/or out-of home placements 

  • Unification and coordination of care to assist with transition to home setting 

  • Weekly family counseling sessions 

  • Treatment is person centered and trauma focused 

  • Family and individual interventions up to 10 hours per week and adjusted based on clinical need 

​Criterion for Services:
​
  • Is one parent or responsible adult with whom the child is living, willing to participate in the therapeutic services, with the goal being keeping the child with the family or transitioning the child home?

  • Does your child(ren)  have difficulty with establishing or maintaining relationships to such a degree that they are at risk of hospitalization or out- of-home placement because of conflicts with family or community?

  • Has your child ever had any prior history of mental health services? 

  • Does your child have difficulty in cognitive ability such that they are unable to recognize personal danger (i.e. drug use, sexual promiscuity, failure to adhere to curfew/runaway, self harm) or recognize significantly inappropriate social behavior? 

  • Are you willing to attend individual and family therapy?

Intensive In-Home Services

Support



A short-term and time limited service that works with children and families that are at risk for out of home placement. Home Based Services requires approval from the local Social Services/Human Services Family Assessment and Planning Teams  in accordance with the Comprehensive Services Act (CSA).

*Services are funded by the Comprehensive  Services Act (CSA) for youth ages 5-21 years old.

​Service Provides:
​
  • 24 hours crisis support and intervention to prevent a higher level of care 

  • Coordination of care with other treatment providers and the FAPT team  

  • Weekly family counseling sessions 

  • Treatment is person centered and trauma focused 

  • Family and individual interventions up to 10 hours per week and adjusted based on clinical need and family participation

  • Customized support based on the family needs

​Criterion for Services:
​
  • Approval and contractual agreement with respective jurisdiction Family Assessment and Planning Team (FAPT). 

  • Parent and/or guardian with whom the child resides is willing to participate in the therapeutic services, with the goal to keep the child in the home. 

  • Identified conflicts in the home, school and community 

  • Willing to attend weekly family sessions and implement behavioral management skills as learned 

  • Services are rendered in the home, unless otherwise clinical noted

Services



Service provides both the individual and family therapy through face-to-face and telehealth clinical sessions. Each prospective consumer is closely paired with a clinician. The outpatient clinician will provide a safe, culturally competent, empowerment approach  to empower individuals and families to work through their life experiences and emotions.

​Service Empowers Families To:

  • Identify, develop and implement healthy boundaries

  • Improve communication and re-define roles within the family system 

  • Improve family dynamics and relationships 

  • Provide strength and coping tools for family members

  • Address dysfunctional interactions and improve family’s problem-solving abilities through a hands on and individualized approach
